血浆致动脉硬化指数对急性缺血性脑卒中患者神经功能缺损程度的预测价值

Value of atherogenic index of plasma in predicting the degree of neurological impairment in patients with acute ischemic stroke

摘要:

目的 探讨血浆致动脉硬化指数（AIP）对急性缺血性脑卒中（AIS）患者神经功能缺损程度的预测价值。方法 收集2022年1月至2024年5月在菏泽市立医院神经内科住院的305例AIS患者的临床资料。根据入院时美国国立卫生研究院卒中量表（NIHSS）评分进行分组：正常组（NIHSS评分0分，86例）、轻度组（1分≤NIHSS评分≤4分，131例）、中重度组（NIHSS评分>4分，88例）。比较3组患者的危险因素的差异，并分析AIP与其他指标的相关性。根据受试者操作特征（ROC）曲线评估AIP对AIS患者神经功能缺损程度的预测价值。结果 3组患者的空腹血糖、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、总胆固醇、AIP的差异具有统计学意义（P<0.05）。AIP与NIHSS评分、空腹血糖、甘油三酯、低密度脂蛋白胆固醇、总胆固醇呈正相关（P<0.05）；与高密度脂蛋白胆固醇呈负相关（P<0.05）。ROC曲线分析显示，AIP预测AIS患者神经功能缺损（NIHSS≥1分）的最佳截断值为2.03（P<0.001），灵敏度为72.1%，特异度为66.3%。结论 AIP与AIS患者的神经功能缺损程度呈正相关性，对神经功能缺损具有一定的临床预测价值。

Abstract:

Objective To investigate the value of atherogenic index of plasma （AIP） in predicting the degree of neurological impairment in patients with acute ischemic stroke （AIS）.Methods Clinical data were collected from 305 patients with AIS who were hospitalized in Department of Neurology， Heze Municipal Hospital， from January 2022 to May 2024. And according to the National Institutes of Health Stroke Scale （NIHSS） score on admission， they were divided into normal group （86 patients with an NIHSS score of 0）， mild group （131 patients with an NIHSS score of ≥1 and ≤4）， and moderate or severe group （88 patients with an NIHSS score of >4）. Related risk factors were compared among the three groups， and the correlations between AIP and other indicators were analyzed. The receiver operator characteristic （ROC） curve was used to investigate the value of AIP in predicting the degree of neurological impairment in patients with AIS.Results There were significant differences among the three groups in fasting blood glucose， triglyceride， high-density lipoprotein cholesterol， low-density lipoprotein cholesterol， total cholesterol， and AIP （P<0.05）. AIP was positively correlated with NIHSS score， fasting blood glucose， triglyceride， low-density lipoprotein cholesterol， and total cholesterol （P<0.05）， and it was negatively correlated with high-density lipoprotein cholesterol （P<0.05）. The ROC curve analysis showed that AIP had an optimal cut-off value of 2.03 in predicting neurological impairment （NIHSS score ≥1） in patients with AIS （P<0.001）， with a sensitivity of 72.1% and a specificity of 66.3%.Conclusions AIP is positively correlated with the degree of neurological impairment in patients with AIS and has a certain predictive value for neurological impairment.
